Sydney claimed the AFL's 2012 Premiership after a heart-stopping Grand Final against Hawthorn that was decided in the final minutes.

In what may be remembered as one of the game's great deciders, the Swans twice had to come from behind to win 14.7 (91) to 11.15 (81).

Veteran Nick Malceski kicked the sealer with an extraordinary left foot snap that sent his teammates and Swans fans into raptures.

5.12pm - Final siren. Sydney are the AFL's 2012 Premiers..

5.03pm - A deafening sigh goes up around the MCG as Jack Gunston - one of the Hawks' most reliable kicks on goal - hits the post from a set-shot. We're back to a six point margin.

5.02pm - Never write off a champion. Adam Goodes has kicked a magical goal to give Sydney a seven point lead - with seven minutes remaining in the game.

5.01pm - A behind for the Swans, which gives them the lead.

4.58pm - Keiren Jack has just levelled the scores for the Swans. It's stating the obvious, but it's impossible to pick a winner from here. Neither side has the ascendency. Every contest is critical.

4.56pm - Crowd attendance at the MCG officially 99,683. Massive crowd witnessing an extraordinary game.

4.52pm - Enter Daniel Hannebery. The young Swan, who won the AFL's Rising Star Award in 2010, kicks a clutch goal for the Swans to cut their deficit to six points - one straight kick!

4.50pm - Another score from Hawthorn, though this time it was only a point. The Swans need to produce something special if they want to win.

4.41pm - David Hale steps up and produces again for the Hawks, with their ruckmen kicking a goal from an extraordinary snap.

4.40pm - Hawthorn have claimed first blood in the final term. Franklin was once again key, laying a tackle that led to an easy goal for Luke Breust.

4.33pm - Three quarter time. Sydney 10.5 (65) to 9.10 (64).

4.28pm - Now it's Sydney's turn in front. Hawks star Sam Mitchell gives away a silly 50 metre penalty to give Jarrad McVeigh a shot on goal. The Swans veteran makes no mistake.

4.26pm - Hawthorn are back in front. A centre clearance paves the way for Isaac Smith to kick truly on the run. Hawks lead by four points.

4.24pm - We're back to a two point margin folks. Do not go anywhere. Lance Franklin has marked and booted a goal from 60 metres.

4.23pm - The Hawks are definitely back! Youngster Jack Gunston shows maturity beyond his years to slot a set-shot.

4.19pm - Another shot on goal for Franklin, but this one slides off to the left side for a behind. Hawthorn are up and running again.

4.17pm - Are the Hawks back? Lance 'Buddy' Franklin boots a team lifting goal after a solid mark against Ted Richards. The margin is back to 16 points.

4.14pm - Third time lucky for David Hale. The big man gives the Hawks a much-needed goal with his third set shot of the match, this one from outside 50 metres.

4.09pm - Sydney are dominating. Lewis Roberts-Thompson kicks another for the Swans. The margin is out 27 points. Hawthorn will need to produce one of the great comebacks to win from here.

4.05pm - Josh Kennedy slots the opening goal of the third quarter after the Swans once again found room through the middle of the ground.

4.02pm - Jarryd Roughead squanders an opportunity to bridge the gap for the Hawks, missing a set-shot.

3.29pm - Morton strikes again! The former West Coast and Richmond player kicks his second for the match to give the Swans a handy 16 point lead.

3.26pm - Mitch Morton extends Sydney's lead with a snap. The Swans' lead is out to 10 points.

3.23pm - David Hale misses another for the Hawks, with a set shot from an angle.

3.20pm - Sydney hits the front after the Swans find space - and spare players - through the middle of the ground. The ball ended in the hands of Sam Reid, who drilled the goal from 50 metres.

3.18pm - Goodes is off the ground, with his knee being tested by medics.

3.16pm - A major injury worry for Sdyney, with Adam Goodes limping after jarring his knee in an awkward landing.

3.14pm - What a start to the term! Sydney get another, courtesy of Jarrad McVeigh. Just one point the margin.

3.10pm - A shocker from the umpire in not paying a free kick for a tackle ends in another goal for Sydney, this one from Kieren Jack, whose father was a rugby league great.

3.06pm - Kennedy answers in the best possible way. An uncontested mark sets up a goal for the Sydney star, whose father and grandfather played for Hawthorn.

3.05pm - The second quarter is underway.

3.01pm - Hawthorn lead by 19 points and the margin could easily have been greater if they had kicked straight. The Hawks largely controlled the opening term. Sydney have much work to do. Josh Kenedy was one of several Swans who were missing in action and will need to lift.

2.59pm - Hawks respond in devastating fashion. Luke Breust kicks his first goal of the match, before Jack Gunston adds another.

2.56pm - Lewis Robertson-Thompson goes from hero to zero. He earns a free kick after tackling Hawks skipper Luke Hodge, but then isses the shot from directly in front.

2.54pm - The play of the day. Lewis Jetta out-runs Cyril Rioli to set up a set shot for Sydney, but the kick at goal didn't go the distance.

2.53pm - Franklin is finally on the board. A mark and set shot from outside 50 metres gives the Hawks a deserved second goal.

2.52pm - Yet another miss by Hawthorn, this time by Jarryd Roughead.

2.51pm - Alex Johnson misses a shot on the run from 50 metres for the Sydney.

2.49pm - Hawks ruckman David Hale takes a mark 30 metres out, almost directly in front, but misses the set shot. Hawthorn are wasting their chances.

2.48pm - Sydney have already made 24 tackles. They average 18 tackles per quarter.
